1. For Student Employees:
- To change your address, complete the
on line Address
Change Form, print, sign, and submit form to the Payroll Office in the Administration Building.
2. For Regular and Contingent Faculty or Staff:
- Complete the on line Address Change Form, print, sign, and submit form to the Human Resources Office in the Administration Building.
- If you have benefits, OHR requires an additional form to change your address for benefits. Link to the OHR web site for the Benefit Change of Address Form.
After receiving your Address Change Form, the Payroll Office will re-route delivery of your check or direct deposit advice to the new address until Central Payroll processes the change.
