Dowell Health Center Patients:

 Patients coming to the Health Center for an appointment will need to complete certain forms before their appointment. It will save time to do this in advance. This may now be done online through our secure patient web portal. The information becomes a part of your confidential medical record. Please click here to access the portal:  MYDOWELLHEALTH

New Entering Student to Towson?

Every new student at Towson must complete the university's mandatory immunization requirements. Go to our secure patient web portal, MYDOWELLHEALTH, and follow the instructions to enter your immunization record into your electronic health record. You will still be required to have your immunization record signed by a health care provider and sent to us for verification. This form can be downloaded from MYDOWELLHEALTH or by clicking here:

Take this form to your health care provider to complete and sign. Either mail, fax, or email a copy to the Dowell Health Center.

  • Consent for Treatment of a Minor (To receive a vaccine or any treatment for routine injury or illness at the Dowell Health Center,  students <18 years of age must have consent from a parent or legal guardian. Print out the form, have it signed by a parent/guardian and return to Dowell Health Center. Fax: 410-704-3715

Need a copy of your Medical Record?

Please print and complete the Consent Form below, then mail or fax a copy to the Dowell Health Center's address and fax number listed above.  Be sure to keep a copy for your records.

 

 

Meningoccal Vaccine/Waiver Form for Housing

Requests for Exemptions from Immunization Requirements: Vaccine requirements are established to protect the health of the student and the entire Towson University community. Only medical and bona fide religious exemptions are permitted.

In the event of an outbreak of a vaccine-preventable disease, students who cannot prove immunity may be excluded from campus by the University Health Officer for the duration of the outbreak. Any resulting withdrawal from classes will be the student's financial responsibility for non-medical exemptions. 

Medical exemptions: Medical basis of exemption request must be documented by the student's healthcare provider on the Immunization form and must be vaccine-specific. If due to pregnancy, temporary exemption will be granted for duration of pregnancy for live vaccines only. 

Religious exemptions: forms available at Dowell Health Center upon request and must be notarized.
