Entrance Health/Immunization form for International Students
Every new student at Towson must complete the university's mandatory immunization
requirements. Go to our secure patient web portal,
MYDOWELLHEALTH,
log in with your Towson NetID and password, and follow the
instructions to enter your immunization record into your
electronic health record.
You will still be required to have your
immunization record signed by a health care provider and sent to
us for verification. This form can be downloaded from the
patient web portal or by clicking here:
PRE_ENTRANCE IMMUNIZATION RECORD.
Don't forget to keep a copy of your signed immunization record
and enter the data in the online Immunization Form at
MYDOWELLHEALTH.towson.edu
