In this document, complete Form A if the student will be living in own apartment or in dormitory. Complete Form B if the student will be living with a US sponsor or relative.

Important Advice: (After receiving your I-20, follow these steps)

• Apply for your F-1 visa early .  The waiting period can be long due to the new security procedures.

• Pay the $200 SEVIS fee; This can be paid through the mail using Form 901, the Internet, or through Western Union.  Be sure to print out and KEEP a receipt as proof of payment.

• Pay the visa application fee. This is usually $100 (US)

• Make an appointment at the embassy or consulate.  Most applicants must have an interview.  This is an important event.  Dress as if you were applying for a job.  Sit up.  Be polite.

• Take all the required documents to your embassy or consulate visit. ( Sevis payment receipt, I-20 from Towson, original copy of financial documents, ELC letter of acceptance, receipt for payment of application fee,  necessary DS forms)

• Always write your name on all forms as it appears in your passport
