ISSO Forms

Please note that it takes 3-5 business days for the ISSO to process requests once completed forms are received.

• CPT Application for Graduate Students
• CPT Application for Undergraduate Students
• Document Request Form - Letter requests must be submitted via our Letter Request Form.
• F-1 Program Extension Form - In order to process this form, we must also receive original, current financial documents valid for at least one year.
• OPT Request Form - In order to for the ISSO to process this form, it must be signed by your academic advisor, and you must first attend an OPT Workshop.
• OPT STEM Extension I-20 Request Form
• Request for Family Member Visa Certificate
• Request for Reduced Courseload
• Social Security Number - Sample Letter for Towson University Employer
• Statement of Willingness (PDF)
• Transfer Eligibility Certificate (PDF)
• Transfer Out Form - In order to process this form, we must also receive your new school's Transfer In form, along with your acceptance letter.
