Happynote is a website that has a lot of interesting music reading video games.  These are sample versions of games that are available to purchase.

Music MasterWorks music editing software: create, play, record, edit and print music with this easy to use music editor. Now with voice-to-note and singing analysis. Record from your MIDI piano/keyboard or use the configurable quick keys and mouse with this music program. Choose between staff and piano views to make your own music, and print both.  http://www.musicmasterworks.com.  There are also some free note reading games.

http://www.solfege.org/ part of open source code project with many different ear training exercises.  Can test intervals, melodies, scales, harmonies and rhythm.

http://musictheory.net/ A nice free site.  Easily customizable.  Can be run from a browser or downloaded for offline use
