printed. You will be notified of the issue number
your submission will be printed should it be
accepted.
Any individuals included in pictures submitted for
publication must provide permission via the photo
release form. Submit
this form with
the appropriate signature(s) to

Use of Images and Recordings for Class Assignments
and Marketing Policy:
Students and faculty must obtain consent before the
taking of images or sound recordings on video tape
film, photograph, audiotape or live transmission for
instruction, scholarship or for marketing purposes.
Procedure:
1. Individuals who can be identified from the image
or recording must sign the university form: “Release
for the use of recorded image or sound on videotape,
film, photograph, audiotape or live transmission”.

2. Images or recordings cannot be taken if the
participant does not sign the form.
3. Signed forms are to be forwarded to the
Department Administrative Assistant or other
appropriate person for filing.
4. Individuals have the right to strike out
statements in the consent form to which they do not
agree or which do not apply before signing the
consent.
