Form Instructions

For a list of the forms you need to submit, you must check your TU Online Services To-Do-List. Some of our forms must be completed online. Many of our other forms are PDF forms that must be printed, signed and submitted to our office.

2017-18 Online Forms

To access our online "Release of Information Form:"

  1. Login to TU Online Services.
  2. Choose Self ServiceStudent Center.
  3. At the bottom of the Finances section choose, Financial Aid Forms.
  4. Choose the appropriate Aid Year.
  5. Choose one of the following forms:
  • Course Load Change Form
  • Housing Verification Form
  • Loan Change Form
  • Notice of Additional Resources
  • PLUS Denial — Request for Additional Unsub (Unsubsidized Loan)
  • Release of Information Form
