Please complete this form if you are unable to enroll in a closed biology course offered for majors in the College of Health Professions during open enrollment for the spring and fall semesters.

Complete one form submission for each closed course in which you need a seat to meet requirements for your major. Please use the comment section to provide any additional information that may help the department assess your need for enrollment. 

Submitting this form helps the department evaluate current demand and assess future need for seat capacity in these courses.
