Forms and Requests

Here you will find all request forms necessary for general needs within the department. Please carefully review the information listed below for more in depth details regarding the respective processes of each form.

Please be aware that all copy requests need to be submitted three days in advance of the date of its requested completion.

The Psychology Department maintains hardware, software, and learning tools for laboratory and classroom applications.

For CLA students presenting research posters, the College of Liberal Arts provides free printing on our HP LaserJet Z6810 large format printer. Please see your faculty member or the administrative assistant of your department for more information.

CLA Poster Printing Submission
Please note that this can only be used by CLA students printing large format posters in conjunction with or for CLA courses/research.  Please submit posters at least two (2) business days prior to your anticipated need.

(FOR UNDERGRADUATE COURSES ONLY)

Complete this form to propose a new course for review by the Psychology Undergraduate Curriculum Committee. This form should be used for both New Catalog courses and new Special Topics in Psychology courses.

The Psychology Department now orders office supplies through Amazon.  The only information needed on this request form is the link to the specific item you would like to order.

Pending budgetary approval each fiscal year, the Psychology Department will fund each full-time faculty member up to a total of $500 for individual professional development funds to advance their teaching, research, and/or service. Funding may be split across multiple purchases, and the grand total is not to exceed $500. The final date for Professional Development Funding requests is May 1st. Please complete the Professional Development Funding Request DocuSign for department review.

All Psychology Department room requests must be submitted through the request form found on this page.

It is important to note that swipe card access requests may take up to 10 business days to be completed.

Parking request should be submitted at least 3 business days in advance.

Requesting Visitor Parking

Before requesting visitor parking through the request form, you will need to have the following information pertaining to your guest: name, email, date and time of the visit, vehicle state/province, and the license plate number of the car being driven to campus by the visitor.
